COVID-19 Visitor Questionnaire

The safety of our visitors, volunteers and employees is a top priority for the Governor, First Lady and the staff of Washington Place.

To prevent the spread of COVID-19 and reduce the potential risk of exposure to our visitors, volunteers, and employees, we are conducting a simple screening questionnaire. Your participation is important to help us take precautionary measures to protect you and everyone at this historic site. All items marked with an asterisk (*) are required.

  • Self Declaration

    If the answer is 'yes' to any of the questions, your visit will be rescheduled.
  • 1. Have you had close contact (within 6 feet for a total of 15 minutes or more) with or cared for someone diagnosed with COVID-19 within the last 14 days?
  • 2. Within the last 14 days, have you taken part in activities where social distancing was not possible such as travel, attending large social/mass gatherings or being in a crowded indoor setting?
  • 3. Have you experienced any cold or flu-like symptoms in the last 14 days (to include fever, chills, cough, shortness of breath, difficulty breathing, fatigue, muscle or body aches, headache, loss of smell, or taste, sore throat, congestion, nausea, vomiting, diarrhea, respiratory illness or otherwise not feel well?
